Common App Parent and Household Information Worksheet

Collect parent and household facts for the Common App Family section and hand a clean list to the student.

Fill the parent-controlled facts, then copy a hand-off list for the student. This page owns one Common App screen: Family / household. It is not another application checklist. For the full document inventory, use the college application checklist. For who enters the data, use the parent-student responsibility map.

Fields below follow official Common App materials dated 25 June 2025 and the current first-year application guide. Labels in the live form can change. Match the application, not this page, if they disagree. Do not add essays, activities, Additional Information writing, FERPA, or aid filing here.

Official sources used for this worksheet: Gather your materials (PDF, 2025-06-25), the first-year application guide, Common App applicant help on limited parent information, and more than five siblings.
