UC Academic History Worksheet for Students With Multiple Schools

Organize UC academic history from multiple schools and college records without inventing grade conversions or duplicate courses.

Use this worksheet for a first pass in 5โ€“15 minutes. Fields are temporary and are not saved when you leave or reload. Copy or print your work before leaving; keep sensitive identifiers and full records outside this worksheet.

Organize UC academic history from the original records of every school attended. Do not enter courses from memory or assume that the current high school's transcript fully replaces an earlier school's or college's record.

UC's application instructions call for high school courses and grades across schools, include college coursework taken during high school, and provide for reporting qualifying middle-school mathematics and language study. Follow the live application's detailed instructions for each situation.

Build a records inventory before typing

Use exact institution and course names where the form requires them. Keep the records beside the application so you can check each entry against its source.

Preserve the record's structure

Identify whether the school uses semesters, trimesters, quarters, or another system. Use the application's directions for recording that structure. Do not average separate grades or convert a grading scale merely to make the entries look consistent.

If a course title is abbreviated or absent from a selection list, use the help instructions and ask support when necessary. A guess can be harder to correct after submission than a question is to resolve before it.

Handle duplication carefully

In a hypothetical case, a college course appears on both the college transcript and the high school transcript. The student should follow UC's instructions for college coursework rather than invent two independent courses or omit the college record. Keep a note of how the entry was resolved.

The same principle applies to a move between schools: report the educational history accurately instead of erasing the first school because the new transcript carries transferred credit.

Review planned work and explanations

Check that senior courses in progress or planned match the actual schedule. Use any explanation field for a concise, relevant clarification when needed; it is not a place to argue about admission chances.

Have the student conduct a final source-to-entry review. A parent can read alongside them and flag mismatches without taking ownership of the application or silently changing grades.

Save the submitted record

Retain the final application copy and confirmation. If you discover an error afterward, follow UC and campus instructions for that type of update rather than creating a duplicate application.

Use the application checklist to track the wider process. IvyReady's undergraduate admissions support can help organize a complicated record, while the student and issuing schools remain responsible for the underlying facts.
